Smith out with injury, Carr in as 49ers’ QB

Niners QB Alex Smith injured his shoulder and left Sunday’s game after being driven into the turf by Panthers DE Charles Johnson.

Smith walked off the field under his own power and headed into the locker room.

Smith has been replaced by QB David Carr.

UPDATE: The 49ers later announced that Smith has a sprained left shoulder and will not return.
